NORTH CUMBRIA INTEGRATED CARE AND CUMBRIA COUNTY COUNCIL GO LIVE WITH ELECTRONIC PATHWAY FOR PATIENTS DISCHARGED FROM ACUTE CARE USING STRATA HEALTH TECHNOLOGY.
(January 2022) Strata Health is pleased to announce the brokerage pathway in Strata Pathways is now live with North Cumbria Integrated Care (NCIC) and Cumbria County Council (CCC).
This innovation will allow Transfer of Care Teams in NCIC to make electronic requests to set up packages of care and/or facilitate short-term care that can bridge the gap as a patient waits for long-term care placements. These electronic requests will source the most appropriate care from approved providers, expediting the care received.
The brokerage team will be able to receive requests, allowing them to then source the appropriate care from CCC approved providers.
Anticipated benefits for providers and patients include:
- Secure transmission of care data.
- Informed decisions around care and appropriate care providers.
- A consistent, fail-safe referral process where no patient falls through the cracks.
From here, Strata Health will work with NCIC and CCC to continually improve this referral process through automation and integration. Next, we will work to implement full integration of the NCIC Patient Administration System to automate data entry into Strata PathWays. Ultimately, this will capture a richer dataset, assisting the brokerage team to make the best possible decisions for patient care.
