NHS Buckinghamshire and Bucks County Council, working in collaboration over the past number of weeks have taken live local authority access to their Resource Matching and eReferral system Strata RMeR. The ability to have Strata RMeR span across both health and social care enables direct bi-directional collaboration that automates the sending, receiving, processing and audit of both section 2 and section 5 documents under the legislated Delayed Discharge Act (2003).

Lynne Swiatczak, Chief Nurse and Director of Patient Care Standards commented, “The vision of automating process to streamline communication across NHS Buckinghamshire including all Buck’s County Council stakeholders is a critical step towards truly integrated health and social care operations. Patients need access to optimal care, regardless of organizational boundaries. We have enabled this in discharge collaboration and are now looking at how the same principles can be applied to added Health and LA collaboration”.

Alison Fabre, discharge lead for the trust added, “We are now able to streamline the discharge process significantly with direct & instantaneous communications from wards to social services in a paper light / paperless environment. This enables case managers to focus on pressure points and escalations whilst the more straight forward process is delivered effectively and efficiently in the background”.

Richard West, ITC Relationship Manager Buckinghamshire County Council and Graham Softely, Associate Director IT for Buckinghamshire Healthcare NHS Trust and their associated teams removed the barriers that had historically been in place to enable specific and targeted system access to Strata by set roles and permissions, delivered across the trusts secure network.
