On the 6th of October 2020, Blackpool Victoria Hospital deployed Strata Health’s patient flow software to manage referrals to specialized cardiac services offered through Lancashire Cardiac Centre (LCC). As one of the leading cardiac centres in the country, the LCC provides complete cardiac care for four hospitals in the North West, from diagnosis to treatment and follow-up. Managing incoming referrals by email, fax and phone caused an unbearable administrative burden on LCC staff, delaying patient admissions, disrupting planning and scheduling of diagnostics and procedures, and ultimately causing preventable patient risks. To complicate matters further, referring hospitals had no visibility into real-time progress or patient outcomes. Now, thanks to technology provided by Strata Health, care coordinators can access a single system to manage the entire referral process. Integrated with the Trust’s existing Patient Administration System (PAS), Strata Health reduces data entry duplication by populating basic patient information into digital referral forms. Offering a single source of truth, Strata Health’s electronic referral form containing the full mandatory data set ensure referrals are accurate and patients are accepted and scheduled into appropriate services without delay.
