
Sub Acute Care — often termed Rehab and Recovery — is in-patient care designed for a client who has an acute illness, injury, or exacerbation of a disease process. It is typically goal-oriented treatment rendered immediately after, or instead of, Acute hospitalization — to treat specific active complex medical conditions or to administer technically intricate treatments in the support of a patient’s underlying long-term condition.
Sub Acute Care also requires the coordinated services of a multidisciplinary team — which can include physicians, nurses, and other relevant professional disciplines — that is trained to assess and manage these specific conditions and perform the necessary procedures.
PathWays Sub Acute/Rehab™ effectively matches specific client clinical needs to available Sub Acute streams of care, ensuring the correct patient care plan and treatment implementation for clinically complex clients discharging from Acute, all while allowing those clients to actively remain on various wait lists for choice facilities once stabilization or treatment course is completed.
