Acute Care is necessary treatment for a brief but severe episode of illness. Most hospitals are Acute Care facilities, where the goal is appropriate treatment, followed by discharge of the patient as soon as they are deemed healthy and stable, and provided with appropriate discharge instructions. Acute Avoidance
By using an objective placement tool, with dynamic prioritization among stakeholders, clients are placed from a multitude of locations, including community, lessening the chance that a patient will degrade and require Acute admittance.

Assessments
Designed for a multi-disciplinary team, our flexible assessment tool allows each member of the team to contribute and edit their contributions to the overall assessment, while providing notification of both uncompleted and completed assessments.

eReferrals
These are electronic referrals within the Acute site, or out to service providers, using matching criteria at a service provider level. Workflow messaging, along with graphical representation of available programs — filtered for appropriateness on a client level — significantly streamline the complex referral process.

Discharge Planning
PathWays Acute Care™ streams clients to specific resources, filtering by appropriateness, client choice, client priorities and regional prioritization. Workflow messaging and user-specific views are designed to streamline and prioritize the discharge process. Logical steps within the placement process contribute to a standard process, with all transactions recorded — providing a solid base for performance analysis.
